One:

will need to print it, fill it out, and fax it to the admissions department at 206-223-1279.

Two:
Fax a copy of
a recent drug and alcohol assessment (done within the last 3 months) to the admissions department at.

Three:
Call to check in with the placement coordinator at 206-223-1300 x234.
For priority placement you must check-in by phone at least
once a week.
